Indications/Uses
Management of RA, short-term symptomatic treatment of acute exacerbations of OA & symptomatic treatment of ankylosing spondylitis. Treatment of juvenile idiopathic arthritis.
Dosage/Direction for Use
Adult OA, RA & ankylosing spondylitis Starting & maintenance dose: 7.5 mg once daily. Max: 15 mg once daily. Childn ≥2 yr Juvenile RA 0.125 mg/kg once daily. Max: 7.5 mg once daily.
Administration
May be taken with or without food: May be taken w/ meals if GI discomfort occurs.
Contraindications
Hypersensitivity. History of stroke (CVA), heart attack (MI), CABG, uncontrolled HTN & CHF NYHA II-IV. History of aspirin- or NSAID-induced asthma, urticaria or allergic reactions. Previous or active peptic ulcer. Perioperative pain in CABG surgery.
Special Precautions
Aspirin sensitivity. HTN, fluid retention or heart failure, advanced renal disease, preexisting asthma. Increased risk of CV thrombotic events, MI & stroke. History of ulcer disease &/or GI bleeding. Concomitant use w/ oral corticosteroids or anticoagulants. Prolonged NSAID therapy, smoking, alcohol consumption & poor general health status. Risk of renal effects. Ensure adequate hydration prior to therapy. Anaphylactoid reactions, hematological effects. Discontinue use at 1st appearance of skin rash or any other sign of hypersensitivity. Monitor for signs &/or symptoms suggesting of liver dysfunction or abnormal liver test. Not for treatment of corticosteroid insufficiency. Pregnancy & lactation. Childn <60 kg. Elderly ≥65 yr or debilitated patients.
Adverse Reactions
Allergic & anaphylactoid reactions, face edema, fatigue, fever hot flushes, malaise, syncope, decrease/increase wt, household accident, edema, fall, flu-like symptoms. CV, GI resp, neurologic, hematologic, urogenital & hepatic disorders. Arthralgia, back pain, dehydration, increased appetite. Dermatologic & sensitivity reactions. Abnormal vision, conjunctivitis, taste perversion, tinnitus.
Drug Interactions
Reduced effect of antihypertensive agents eg, ACE inhibitors, β-blockers & vasodilators. Increased clearance w/ bile acid sequestrants eg, cholestyramine. Increased rate of GI ulceration w/ aspirin & other NSAIDs. Increased nephrotoxicity of ciclosporin. Reduced natriuretic effects of furosemide & thiazides. Increased plasma lithium levels. Enhanced methotrexate toxicity. Increased risk of bleeding w/ oral anticoagulants eg, warfarin, antiplatelet drugs, thrombolytics & systemic heparin.
